History of disc prolapse 7 years ago on conservative treatment
Patient complains of severe back pain 3 days ago.
Patient has history of recurrent mild epistaxis related to sun exposure. Attacks always stops spontaneously. Last attack was 3 days ago.
Patient gave history of low platelet in previous routine investigations.
WBC: 10.4X10³/uL
HGB: 14.4 G/dL
MCV: 82.5 fL MCH: 26 PG
MCHC: 31.5 %
PLATELET: 41X10³/uL.
PT: 12.3 sec (INR: 1.04)
APTT: 29 (R.R.: 25-36)
Normal RFT, LFT and fasting Blood sugar
Platelet functions : Not available in our center.
Neutrophils show basophilic cytoplasmic inclusions ; resembling Dohle bodies. These inclusions are also seen in eosinophils and monocytes
Severe thrombocytopenia; giant platelets are frequantly seen.
Picture is highly suggestive of May-Hegglin anomaly (to be confirmed by molecular study: MYH9 gene mutation)
Patient refused admission. He left against medical advice (with consent)
This patient is a brother of the suspected May-Hegglin anomaly patient discussed in case No. 12 in this atlas.
