38 year old male

Clinical Data

No medical history of chronic disease (Medically free)

Complain: Recurrent discharging swelling in the right foot increasing gradually over 1 year. Swelling is not associated with fever or weight loss

Examination: Generally patient looks well and clinically stable.

Right foot examination: Swelling with sinuses have bloody and purulent discharge associated with white granules.

Related Laboratory Results

Crushed granules are sent to the microbiology lab for gram stain and KOH examination.

Comment / Findings

Gram stain (1st panel): Filamentous branched gram positive bacteria - non acid fast.

KOH Preparation (2nd panel): Fine branching filaments, unstained with lactophenol cotton blue.

Final Diagnosis

Actinomycetoma
